S. F. Johnson Consulting Claim Input Instructions & Handling Guidelines
In the states of Connecticut, Delaware, Kentucky, North Carolina, New Hampshire, New Mexico, South Carolina, Texas and Vermont we are licensed to adjust claims. In all other states, at this time, we perform oversight only. New states are being added monthly as it is our quest to have the ability to adjust claims in every state across the US.
Whether the claim takes place in a state where we are licensed or otherwise, we handle each loss with standards better than those required in each state.
You will receive a confirming email within 30 minutes of claims submission. You will receive a preliminary claims update within 24 hours of confirmation. This update will include your unique incident number, the facts of the loss as we have received them from you. We will include a bullet list of the information required to move the claim along. Depending on the claim type, we may need this information immediately or within 30 days.
Information Flow & Guidelines
You will receive responses to all correspondence within 48 hours of receipt.
Claims reports are generated every 30days and emailed to you at the email address provided when the claim was submitted.

Company Name
You may only input claim information if you are an existing S.F. Johnson Consulting Client. If you are, please input your complete business name. If you are not a S. F. Johnson Consulting client and would like for us to handle or oversee a claim on your behalf, please contact our Customer Service Manager by email and she will start you on your way to becoming a client and we will begin handling your loss.

Please indicate the name of the person reporting the claim.
Email
Please provide us with the name of the person you would like to receive our claim confirmation.
Loss Date
Click on the calendar to indicate the date of the loss.
Loss Type
Please indicate the type of loss. This assists us early in the game and lets us know what type of information we should be looking for and how we should direct the investigation.
Loss facts
Let us know what you know about this claim so far. We will get the rest!
Was anyone injured?
This is very important to know. Knowing this up front also better directs the investigation.
Claimant Information
Please tell us all you know about the other party or parties involved. Please include all addresses, phone numbers, email addresses. Please include the other party’s insurance information if you have it.
Damages/Injury information
Tell as much as you known to date about the damage to the property or injury to the person.
If this is a 1st party property claim, please tell us all you can using this form. For all additional damaged/missing property, email that list to claims@sfjohnsonconsulting.com. Please indicate "NEW PROPERTY LOSS INFO" in the subject line.
